Adoption Agreement and ReleaseI/We, the undersigned Adopter(s) understand and agree to the following terms and conditions of this contract to adopt the aforementioned dog, further described within this document.I/we understand that non-compliance with the terms of this agreement gives Braveheart Bulliez Rescue, Inc. (hereafter “BBR”) the right to reclaim this dog without refund of the adoption fee or other compensation.

1. The dog will receive proper food and water, shelter, and loving humane treatment by my- self and other family members, and will never be left alone for extended periods of time (more than 6hrs/day).

2. The animal will never be chained outside or be kept as an “outside” dog for the life of the dog.

3. I agree that the dog will wear an ID tag affixed to a non-slip/martingale collar or harness at all times.

4. I agree to abide by all state and local animal control and leash laws. I understand it is my responsibility to become familiar with these laws and to license the dog according to all regulations.

5. I understand that BBR has limited knowledge of the animal, including medical issues beyond a regular check-up and standard shots given by veterinarian, background and disposition of the animal. BBR makes no warranties in regards to the health of the dog and disclaims any liability for the dog’s medical condition, sickness, or disease.

6. When I assume ownership, the dog will receive annual vaccinations according to a yearly veterinary schedule, and will be treated immediately for any illnesses or injuries that may occur.

7. When I assume ownership, the animal will receive consistent, monthly heartworm preventative AND flea/tick preventative.

8. The dog will not be subject to harassment by myself or other persons who live in or visit my home, and will be protected from other animals in my care. Additionally, the dog will not be used for purposes of experimentation, vivisection, or breeding.

9. I understand that a BBR representative may inspect my home and the animal, upon their request.

10. I release BBR, its Board of Directors, volunteers, foster homes and representatives, and the veterinarian treating the dog, from any and all claims and actions, whether for property damage caused by the animal(s) or for personal injury to me, family members and other animals in my care (personal pets or livestock) from biting, scratching, transmittal of disease, and/or any other matter attributable to said animal(s).

11. BBR cannot be held accountable for the dog showing any signs of aggression if we were not aware of it before the dog leaving. Any and all behavior observed at the rescue will be fully disclosed to the adopter prior to the adoption being finalized. We understand that sometimes dog's temperaments change in different situations. All adopted dogs should be allowed a decompression period to adjust to new homes, persons and situations.

12. If dog is unaltered at the time of adoption, adopter agrees to have dog altered within sixty (60) days of adoption. Proof of alteration must be submitted to BBR no later than 60 days post-adoption. Certain breeds may require alteration at an older age due to medical reasons or growth plate maturity.

I agree to pay the adoption fee of $375.00 for dogs over the age of 9 month old and $475 for puppies who are 9 months or younger. The fee is due immediately upon signing of this agreement and receipt of medical records, since dog is already in your possession.

I understand and agree to the above terms. If, at any time, I am unable to care for the dog according to the conditions stated above, I will allow Braveheart Bulliez Rescue to resume ownership of the dog and will not sell, give away, or otherwise dispose of the animal.

I understand that if I knowingly fail to comply with the above terms, or in any way put the animal in a situation which endangers its health, Braveheart Bulliez Rescue may take appropriate action, including removing the pet from my possession and/or taking legal action against me.

‍I understand that if any misinformation, omission, or untruth is discovered on my adoption application after an adoption takes place, Braveheart Bulliez Rescue reserves the right to annul the adoption and reclaim the dog immediately.
